Setting the Standard for Lactation Equity in Higher Education
UCSF’s Lactation Accommodation Program is a shining example of what’s possible when institutional support, structural change, and community care come together. Under the leadership of Caroline Carter, IBCLC, the program has grown into one of the most comprehensive lactation support systems in higher education—serving over 300 employees and learners annually across 13 campuses.
Since its inception, the program has expanded access to 100+ lactation rooms, free Ameda pump kits, cooler bags, discounts on supplies, and access to the Milk Stork platform for employees who need to ship milk home while traveling for UCSF business. Monthly “Pumping & Returning to Work” workshops create a space for community-building and personalized support, while manager toolkits and supervisor trainings ensure that leadership understands and champions lactation accommodations at every level.
The data reflects the program’s impact:
95% of 2025 survey respondents agreed the program positively enhanced their pumping experience at work or school.
Over 960 ounces of breastmilk have been shipped since launching the Milk Stork portal in 2024.
The number of lactation rooms has quadrupled in less than a decade.
More than just space and supplies, UCSF is shaping policy. The program played a central role in revising PPSM 84, a UC systemwide Lactation Accommodation Policy that went into effect in January 2025. Through their Lactation Accommodation Advisory Group, UCSF continues to identify and act on areas for improvement—centering learners, expanding pay for pumping, and prioritizing equity in every corner of campus.
One employee said it best:
“I’ve worked in many different hospitals, and I have to say that UCSF is one of the most supportive when it comes to pumping! I really appreciate all the resources that are in place.”
By combining structural solutions with heartfelt support, UCSF is helping parents achieve their feeding goals—and raising the bar for institutions everywhere.
