Join NAACHO on January 12, 2023, from 1:00-2:30 PM ET, for the sixth webinar of the 8-part “Every Step of the Way through the 1,000 Days” series, on Relating to the Family Unit and their Social Determinants of Health in Chest/Breastfeeding support, that will focus on Recommendation 6 of the Continuity of Care in Breastfeeding Support: A Blueprint for Communities.

This session will discuss relational continuity, cultural humility, family-centered lactation care, understanding the intersectionality of families, and how social determinants of health impact an infant’s feeding journey. Relational continuity, one of the dimensions of continuity of care, refers to the interpersonal aspect of the family unit and provider relationship, including the support and encouragement to the family unit through the development of trusting relationships between families and the professionals providing lactation support and other services.

Lowell Community Health Center, will present their overall historic community work rooted in deep ties to culturally responsive approaches, including their breastfeeding continuity of care work in partnership with their Community Health Worker, and their work with food pantries and with urban agriculture. Thames Valley Community Action Agency, is a community leader in advocating for and meeting the needs of their community by providing wraparound services to families. They will discuss how they are advancing continuity of care in their community, while addressing many social determinants of health needs for their families through their No Wrong Door approach.
